Visa application form is obligatory registered online on the website www.visa.mfa.gov.ua

Visa application questionary consists of 36 points that shall be filled accurately and precisely.

Tip 1. Read the question before answering.

Tip 2. All the passports’ data filled in the application form shall strongly correspond to the passport.

Please, verify twice each letter and number while filling in the information about your surname, name, date and place of birth, passport`s series and number, dates of validity and authority that issued.

Tip 3. Point 16: Applicant`s postal address and email address.

Postal address shall be filled in accordance with the postal rules of your country, including postal index (if applicable), country, province, city, district, street, number of building and apartment.

Email address shall be active in case if we will need to contact the applicant.

Tip 4. Point 17: Telephone number including international dial codes.

✔Please, ensure that number is valid and active. Applicant shall write personal phone number or in case of absence of the personal - the phone number that the Embassy will be able easily contact with the applicant.

❌Please do not include in the visa application form the phone numbers of strangers, friends of relatives, neighbors etc.

Tip 5. Point 19: Current occupation. And point 20: Employer`s name, address, telephone and fax number, email address.

✔Please, write only your current, factual occupation.

❌Do not write the occupation of your wish or dream.

All the data shall be correct and valid. Please, ensure that the phone number and email address are active.

Tip 6.  Purpose and duration of travel shall be confirmed by the relevant documents.

For example, the invitation for study in case of study visa shall be valid at least for 30 days more after the day of your entrance on the territory of Ukraine.

Tip 7. While filling in the intended date of arrival and departure keep in mind that normally visa process takes 10 working days (until 30 days if necessary).

Tip 8. Please, ensure that you have properly filled in points 29, 30, 31, 32 of the application form.

✔The applicant shall know and write in the application form the address of residence in Ukraine in accordance with the postal rules, including postal index (if applicable), country, province, city, district, street, number of building and apartment, telephone number and email address.

✔The applicant shall know and write in the application form all the valid and correct data of the inviting person/entity in Ukraine.

Please ensure that you are providing us with the direct, correct and active phone numbers and email addresses of the inviting person or entity.

It’s better not to write the abstract data of “you-don`t-know-who” because in this case the inviting person or entity will not be able to confirm your invitation. This is the most common mistake.

Tip 9. Read all the information that is written and ensure that you understand the information before signing the application form.

Tip 10. Do not forget to choose the day and time of your application to the relevant visa center. Without this information the application form is not recognized by the system as properly registered.
